The end of the school year is quickly approaching. Arrangements need to be made before May 15th to pick up remaining medication(s) or medical supplies from the campus nurse.
It will be assumed that any medication/medical supplies not picked up are not wanted and will be properly disposed of. Medications/medical supplies CANNOT be sent home with students or kept in the clinic for the next school year.
**Medications require a new CVISD Medication/Treatment form signed by the physician (prescription) or parent/guardian (non-prescription/over-the-counter OTC) each school year.
